The main purpose of this section is to call to attention, things that must be done by a survivor after a death. By making taking these important step now, and organizing this information, you can minimize the emotional strain that will be placed on your survivors.

  • Social Security Number
  • Date of Birth
  • Place of birth
  • Citizenship
  • Father's name
  • Father's birthplace
  • Mother's maiden name
  • Mother's birthplace
  • Religious name (if any)



Documents to Collect
  • Will
  • Birth Certificate
  • Social Security Number
  • Marriage license
  • Citizenship papers
  • Insurance policies (life, health, automobile, property)
  • Bank books, Check Accounts, IRA's, etc...
  • Deeds to property
  • Income tax returns, receipts or cancelled cheques
  • Military discharge certificate
  • Disability claims
  • Cemetery Certificate of Ownership
  • Loans/Mortgages ; House, Car, etc...
